Montecatini Alto is the medieval town, located on the tip of the hill for defensive purpose. The well-preserved city walls have six gates and run around the Romanesque central fortress.

| Parish of St. Mary of the Assumption |

St. Mary of the Holy Assumption is dedicated to the Patron Saint and is the local parish, placed in People Square. It is an interesting modernist building formed with striking repetitive angular geometries and featuring an almost plain dome set on a thick drum.
